The following are some of the most commonly used application forms. You may print them off and send them to the appropriate Electoral Registration Officer. The forms include the addresses for the Perth & Kinross Council and Angus Council areas, but you can send them to the Electoral Registration Officer for another area if your registration address is elsewhere. • application by a postal proxy to change ballot paper address at a particular election (PO6)
